
The heart of Service Coordination is helping individuals define and realize their goals, needs and desires as outlined in their Individual Service Plans. Service Coordinators work collaboratively with the individual, their family and other support providers in the design of such a plan. The Service Coordinator helps obtain access to needed services, advocates on behalf of the individual and works with the individual’s network of supports to enable him or her to increase the skills necessary to live a more independent and fulfilling life.
Service Coordinators – Utilizing a Person-Centered Planning Approach
To help achieve the personal goals, preferences and needs of the individuals we support, Lifetime Assistance Service Coordinators utilize a holistic and person-centered approach to service management.
- Each Service Coordinator initiates the process with an interview
- Then, the individual with a developmental disability selects his or her own Service Coordinator
- The Service Coordinator serves as a support partner, following the individual and his or her family the entire time they attend Lifetime Assistance and beyond
Lifetime's Service Coordination Department serves over 530 adults and children with developmental disabilities throughout Monroe, Genesee and Orleans counties. Our Service Coordinators conduct visits with each person at least monthly to ensure that services and supports are satisfactory.
Service Coordinators Provide:
- Person-centered planning
- Crisis intervention assistance
- Referrals for Residential, Day and Vocational Services
- Connection to medical services and psychological counseling
- Advocacy
- Long-term follow up
Jefferson School & Edison High School Collaborations
A Lifetime Assistance Service Coordinator is located at both schools to provide linkage and referral services to community organizations for children who attend either school, as well as their families.
Our Service Coordinators:
- Act as liaisons between the school, the student and the parents
- Attend CSE meetings and help the communication process between school personnel and parents
- Conduct home visits upon receiving referrals from school personnel or students to assist with such things as school attendance, behavioral concerns, relationship problems with other students, etc.
- Help qualified families enroll in programs that assist with food, shelter, clothing and other necessities

Comprehensive Parent Training
This program provides intensive training and supports to parents with developmental disabilities. The program is designed to train parents in the areas that are specific to each parent’s goals and needs. A Family Coordinator is assigned to work with each family in their home. The Family Coordinator and parents will develop person-centered goals that assist with parenting. Some of the goals that parents choose to work on include anger management, proper disciplinary techniques, medical needs, nutrition, advocacy and understanding the stages of child development. This program is designed to empower parents with developmental disabilities and support their ability to care for their children in a safe and healthy manner.
